Handovers get written on the last afternoon, from memory, as a long list of everything the outgoing person knows. The new owner gets a lot of detail and still does not know what is due next week, who to ask for the banking token, or which supplier is in a dispute.
Process Handover is organized around the calendar of the process. It starts with the next three due dates, then writes an ownership map, an access and contacts table, the live state of unfinished work, and the history that matters. It ends with a scheduled walkthrough and a read-through as the new owner.

How is this different from just asking Claude directly?
Asked cold, Claude produces a generic handover template. This one is built from your process and its next due dates, puts the perishable in-flight work first, and ends by reading the document as the new owner to catch every sentence that would trigger a question.
How is this different from a Tool?
